468 Use of calcitonin gene-related peptide monoclonal antibodies in patients with rosacea: An exploratory, comparative case series

Rosacea is a chronic inflammatory skin condition with neurovascular components. Calcitonin gene-related peptide (CGRP) hypothesized to play key role in the pathophysiology of rosacea, but evidence remains limited. In this study, we aimed describe effect CGRP monoclonal antibodies (MAbs) on rosacea signs and symptoms. We conducted retrospective cohort study at single academic center from 2016-2022 comparing symptoms before versus MAbs. Patients were followed for mean duration 1.2 years prior starting MAbs 1.0 while Our consisted adult patients who started (galcanezumab, erenumab, fremanezumab) least 2 months treatment migraines. Outcome measures binary, either presence or absence (papules/pustules, erythema/flushing, ocular symptoms, burning/stinging) treating physician's overall assessments as documented medical records. Type severity MAb-related adverse events recorded. Of 13 patients, 7 (53.8%) experienced papules/pustules after MAb initiation, 23.1% had continued papules/pustules, only 7.7% worsening papules/pustules. Erythema/flushing became absent 53.8% MAbs, 15.4% erythema, developed erythema Six (46.2%) improvement based physicians’ worsening.
